Patients need to bring the following to their appointment:
- New York Medical Marijuana Program Registry Patient ID Card
- Certification Form signed by a registered practitioner
Please note, at present we accept cash and debit cards as payment types.

If you are a caregiver, please bring your New York Medical Marijuana Program Registry Caregiver ID Card. If possible, please also bring the patient’s medical marijuana ID card to the initial consultation as well. This allows us to scan both cards and link them as one file in our system.
